Overview of Scribing Services
Scribing Services support the documentation and maintenance of clinical records by a physician. Physicians can utilize ancillary personnel, typically experienced scribers with clinical documentation expertise, to document/record the work performed by a physician in either an office or a facility setting. The scribe is not a care provider and does not act independently. They do not provide any clinical items or care services.
The scribe focuses on documenting the physician’s dictation and activities during the visit. The documentation is performed either on structured patient charts or as Electronic Health Records on an EHR platform. The scribe performs just the documentation on the treating physician’s / non-physician practitioner’s signature, which indicates that the physician/NPP affirms the documentation of the care provided.

Real-time scribing tools. We see increased adoption of Google Glass and other audio/video Communication tools to perform scribing as there is more bandwidth available today.
Natural Language Processing Documentation. Technologies such as digital clinical assistants assist scribes in the strenuous task of medical documentation.
